You have probably heard the usual advice. Get 150 minutes of moderate exercise a week, or 75 minutes of hard exercise, and you have done your part. That advice is fine as far as it goes. The trouble is that it only speaks to a small slice of your week and says nothing about the other 23 hours of your day.
So what about the rest of the day? What about the person whose knees will not tolerate a treadmill, or who sits at a desk from morning until evening, or who simply cannot get to a gym?
A study published in July 2026 in the journal BMC Medicine set out to answer exactly that, and what it found is genuinely encouraging. Researchers had 59,218 adults in the United Kingdom wear a movement tracker on the wrist for a full week, then followed their health records for about 8 years afterward. During that time, 2,385 of them were diagnosed with cancer. By comparing how people spent their days against who did and did not get sick, the researchers could ask a very practical question.
The Question They Asked
Here is what makes this study different from most.
You only get 24 hours. You cannot add time to one activity without taking it from another. If you stand for 30 more minutes, those minutes came from somewhere, most likely from sitting or from sleep.
So instead of asking “is exercise good for you,” which we already know, the researchers asked something far more useful: if you took 15 minutes away from sitting and gave them to walking, what happens to your cancer risk? What if you gave them to standing instead? What if you took them from sleep?
They sorted every minute of each person’s day two different ways. First by posture, meaning sleeping, sitting, standing, or moving. Then by effort, meaning sleep, sitting, light activity, moderate activity, and hard activity. Then they looked at cancer of 13 different types already known to be connected to how active people are, including colon, breast, lung, kidney, liver, and bladder cancer.
The Average Day Was Sobering
Before the results, look at how these people actually lived. The typical participant was about 62 years old. On an average day, that person slept 7 hours and 39 minutes, sat for 11 hours and 13 minutes, stood for 4 hours and 17 minutes, and moved for 51 minutes.
Fifty-one minutes of movement in a whole day.
Sorted by effort, the same average day included about 5 hours of light activity, 28 minutes of moderate activity, and 3 minutes of anything you would call hard.
The people who later developed cancer had spent less time moving and standing, and more time sitting, than the people who did not. Sleep length made no real difference either way.
What They Found
Fifteen minutes of moving is a real, measurable thing. Trading 15 minutes of sitting for 15 minutes of moving was linked to about 2% lower cancer risk. So was trading 15 minutes of sleep for moving. Two percent sounds small, and for one person it is small. But this is a change so modest that almost anyone could make it, and it is one you can repeat every single day for the rest of your life.
Standing counts too, and that is the news here. Simply being on your feet, not walking, not exercising, just standing, was linked to lower risk. It took about twice as long as walking to get the same benefit, roughly 30 minutes of standing instead of sitting or sleeping. But it counted. The researchers say this is the first study of its kind to show that trading sitting time for standing time may help. They are appropriately careful and call it a promising lead rather than proof.
Going the wrong direction hurts more than going the right direction helps. This is the part I would underline. When people had 30 minutes less movement in their day, and 30 more minutes of sitting, their risk was about 8% higher. Thirty minutes of movement given up for extra sleep was about 7% higher. Trading standing for sitting raised it about 3%. The pattern was not a straight line either. The more hours a person sat, the faster the risk climbed. In plain terms, protecting the activity you already have may matter just as much as adding new activity.
Short bursts of hard effort do the most per minute. Just 3 extra minutes of vigorous activity a day, taken from any other part of the day, was linked to about 4% lower risk. Three minutes. To get that same benefit from gentle activity like slow walking took more than 90 extra minutes. Both work. One is simply far more efficient with your time.
Hard effort is not just “more” moderate effort. When the researchers looked at moderate and hard activity together, the benefit kept improving as the hard portion grew. That suggests the two are doing somewhat different things in the body, and that a little genuine intensity mixed into your regular activity adds something that more moderate walking alone does not.
Why This Might Work
The researchers point to a few likely explanations. Movement lowers chronic inflammation and improves how the body handles insulin and blood sugar. Long stretches of sitting appear to push both in the wrong direction. Standing and light activity may help mostly by breaking up those long stretches of sitting rather than through any effect of their own.
One finding is especially worth noting. When the researchers accounted for body weight, the connection between sitting and cancer disappeared. That suggests much of the harm from sitting may work through what it does to body composition and metabolism over time. This lines up with what we see in practice, and it is one more reason to treat metabolic health as central to cancer prevention rather than as an afterthought.
What the Study Cannot Tell Us
Honest research says what it does not know, and this paper does.
This study compared different people to each other. It did not follow the same person as they changed their habits. So it shows a connection, not proof of cause and effect, and it cannot promise that starting to stand more today will lower your personal risk.
The benefits from short bursts of hard activity became less certain when the researchers set aside cancers diagnosed in the first two years, though the broader activity findings held up in every version of the analysis.
Breast cancer showed no connection at all in this group, even though it was the most common cancer here. The researchers flag that directly and caution against assuming every cancer type responds the same way.
People who volunteer for studies like this tend to be healthier than average, and 96% of this group was white, so the findings may not apply equally to everyone. A wrist tracker also cannot tell whether someone was standing at a job or standing by choice, and those may not be the same thing.
The study was funded by Cancer Research UK and an Australian research council. One of the senior authors discloses that he consults for and holds a stake in a company working in this area.
What I Would Take From This
The practical message is refreshingly simple.
If your body allows it, work 3-5 minutes of genuinely brisk effort into your day. One hard flight of stairs. One uphill stretch on your walk. Something that leaves you actually breathing hard for a few minutes. Per minute spent, nothing else in this study came close.
If that is not possible for you right now, you are not without options, and that is the real gift of this research. Fifteen minutes of ordinary moving instead of sitting counted. Thirty minutes of standing instead of sitting counted. You can stand during a phone call. You can stand while you cook. You can raise your desk.
And whatever you are already doing, protect it. The clearest signal in this entire study was how much the risk rose when movement was traded away for sitting.
We were designed to spend our days upright, varied, and in motion, with rest in its proper place. What this study shows, with unusual care and detail, is that the further we drift from that design, the more risk quietly accumulates. The hopeful half of the finding is how small the corrections need to be.
